Community Pathways Waiver — General FAQs
How Services Start
We make starting services clear and straightforward. While each individual's situation is unique, the process typically follows these steps:
- 1
Referral or Inquiry Received
A referral is submitted by a family member, Case Coordinator (CCS), Support Broker (SB), or community partner, or an inquiry is made directly to HAVE Support.
- 2
Eligibility & Service Review
We review eligibility, program enrollment (DDA or Autism Waiver), and the requested services to confirm alignment with authorized supports.
- 3
Coordination With the Support Team
HAVE Support coordinates with the individual's CCS, Support Broker, Service Coordinator, and family to clarify needs, timelines, and documentation.
- 4
Service Authorization
Services begin once authorization is confirmed and all required approvals are in place.
- 5
Service Start
Supports are scheduled and services begin in accordance with the approved plan.
