DDA Application Checklist

Use this checklist to track where you are in the DDA application process.

• Gather birth certificate and Social Security card
• Collect recent medical records documenting the disability
• Obtain proof of Maryland residency
• Apply for Maryland Medicaid eligibility first
• Submit the DDA application (available at health.maryland.gov/dda)
• Request confirmation that your application was received
• Complete the DDA eligibility interview with your coordinator
• Participate in the Person-Centered Planning (PCP) meeting
• Review and sign your Person-Centered Plan
• Choose a service provider and begin services
